1What must be available when management says an investigation is complete?
For each applicable incident in the last 12 months, its investigation report or a summarised lessons-learned bulletin.
2Does the PDF set a minimum near-miss quota?
No. It asks the inspector to record the number submitted in the past 12 months and expects reporting/investigation procedures.
Where a finding may arise
RISQ explicitly requires a finding for an incident not documented in the RightShip 24-month record. Missing completed-investigation reports/lessons or ineffective reporting and closure are also assessed.
