Check the vessel’s actual procedures, equipment and records against these points from RISQ 3.2.

Provide and implement a PPE matrix based on task risk, suitable standards and manufacturer instructions.Provide safety harnesses and fall-protection devices and make their use mandatory in the SMS for aloft/overside work. RightShip recommends an inertia-reel fall arrestor with a full-body harness.Have fall-protection devices examined by a competent person every 12 months or after a fall, to maker instructions; retain records.Minimise cargo-dust exposure; provide suitable respiratory protection and goggles where hazardous solids/liquids/gases cannot be avoided. Match filters to the hazard and renew per maker guidance, or when soiled if no guidance.Provide and wear full task-appropriate PPE throughout hold cleaning using chemicals or specialist equipment; define minimum PPE for rigging a combination pilot ladder overside.Provide suitable electrical protective equipment for exposed body parts and work; maintain helmets to maker instructions and replace after significant impact or loss of required protection.If ammonia is used as fuel, provide suitable recognised-standard protective equipment including eye protection for fuel-system work, in clearly marked accessible lockers.

1Is availability of a harness alone enough?

No. RISQ also checks that the SMS makes fall protection obligatory for aloft/overside work and that devices receive the specified competent inspections.

Where a finding may arise

RISQ explicitly requires a finding where a safety harness/fall-protection device is absent or the SMS does not require its use aloft/overside. Unsuitable, unmaintained or unused task PPE is also assessed.
