The plan, log or checklist could not be reconciled with signed line-up checklist, cargo plan, piping diagram, valve positions, deck inspection and any later change record.
The response did not adequately control same-person self-check, desk verification, wrong valve, open crossover, missing blank, closed vent path, open drain or unrecorded change.

Use the latest real operation, not a model checklist. Follow one critical parameter from the agreed plan to the CCR display, deck condition and final record.
