Where an observation may arise

Inspection identified wrong line, passing valve, damaged flange, trapped liquid, missing blank, low-rated fitting, poor support or unreadable label.
Records did not prove valve and ESD operation, leak test, blank and drain condition, gauge verification and line-identification check through the complete operating path.
An alarm, trip, interlock or indication was isolated, incorrect, overdue for test or not understood.
PMS, calibration, service or repair evidence was missing, overdue or not traceable to the item.
Responsible personnel could not explain line identification, material and pressure rating, valves, blanks, drains, ESD valves, insulating arrangements, supports and spill or drip control, operating limitations or failure response.
Master's practical note
Trace one signal or flow path from the field device to the CCR or ECR and back to the final protective action. A successful local test alone may not prove the full system.
