Where an observation may arise
The fitted system, drawing, manual or certificate did not agree.
Inspection identified high oxygen, soot carryover, corrosion, scrubber failure, burner instability, failed trip or gas leakage.
Records did not prove start-up purge, oxygen and pressure response, alarm and trip functions, blower performance and deck supply through the complete operating path.
An alarm, trip, interlock or indication was isolated, incorrect, overdue for test or not understood.
PMS, calibration, service or repair evidence was missing, overdue or not traceable to the item.
Responsible personnel could not explain combustion, cooling and scrubbing, oxygen analysis, blowers, deck delivery, shutdowns and alarms, operating limitations or failure response.
Master's practical note
Trace one signal or flow path from the field device to the CCR or ECR and back to the final protective action. A successful local test alone may not prove the full system.
