Past observations for practice
During the ER round it was noted that the FO pump and FO flow controller of the boiler were operated in the manual mode.
A dirty rag was found below FO purifier feed pump No. 2.
The OP was the designated safety officer as per the company procedures. Safety inspections were reportedly carried out by the OP but was unaware of the checklist used. This was later produced after consultation with the attending supdt.
Master's practical note
Walk the latest inspection route with the report. If obvious defects are absent from the record, question the inspection method before adding more checklist items.
