Where an observation may arise

FALSE COMPLETION

A job is closed without evidence that the work or test occurred.

UNCONTROLLED OVERDUE

Critical work is overdue without approval or compensating control.

MISSING EQUIPMENT

Fitted machinery or a statutory task is absent from the system.

DEFECT HIDDEN

A temporary repair or recurring failure is closed administratively but remains onboard.

SPARES MISMATCH

Recorded critical stock differs from physical quantity or condition.

INTERVAL ERROR

The PMS conflicts with maker, class or statutory requirements.

Sample one recently completed critical job at the machine. The work report, fitted condition, used spares and test result should form one consistent story.

Equipment boundary and references

Equipment boundary: The PMS may be electronic or paper-based, but company approval and access control must be clear. Deferral does not change a statutory or class due date without the required authority.
